Please bring your insurance card (if applicable), a valid photo ID, a list of any medications you're currently taking, your current eyeglasses or contact lenses, and any previous eye exam records if available. New patient forms are available to complete before your visit to save time at check-in.

Absolutely! We welcome patients from 6 months of age and have specific experience in pediatric eye exams. The AOA recommends a child's first eye exam at 6–12 months, then again at age 3 and before starting school. Early detection of vision problems is critical for healthy development.

A comprehensive eye exam at Arizona's Vision typically takes 45 to 90 minutes, depending on whether additional tests like Optomap retinal imaging or contact lens fittings are included. New patients may want to allow a bit of extra time for check-in paperwork.

We are located at 15215 S 48th St STE 180, Phoenix, AZ 85044. There is ample free parking available in the building lot. Our location is easily accessible from the 202 freeway and is convenient for patients coming from Ahwatukee, Chandler, Tempe, Gilbert, and surrounding communities.
